BOPPN WAIVER + CONSENT FORM I am not aware of any injury, illness, or other health related issue that would restrict or limit my ability to participate in Gaggle.go’s Boppn Scavenger Hunt Event (the “Boppn Scavenger Hunt Event”). ASSUMPTION OF RISK / WAIVER / RELEASE / INDEMNIFICATION I understand and fully and completely accept that there are inherent risks involved in participating in the Boppn Scavenger Hunt Event, and that accidents, injuries and damage to person and/or property may occur during the event. Accordingly, I hereby knowingly and freely accept and assume any and all such risks, both known and unknown. In consideration of my being permitted to participate in the Boppn Scavenger Hunt Event (and related events and activities), I hereby covenant not to sue and to forever release Gaggle.go LLC (dba Boppn) and/or any of its owners, members, officers, employees, agents, volunteers, other participants, sponsors and advertisers (collectively, the “Released Parties”), from any and all present and future claims, damages, injury, disability, death and loss or damage to person or property (whether resulting from negligence or any other cause), resulting from my participation in the Boppn Scavenger Hunt Event and/or any other activities related thereto, when or however the same may occur. To the fullest extent permitted by law, I hereby waive any and all claims resulting from any cause (including negligence) that may (or may otherwise have been) made by me, my family, estate, heirs and assigns. I further agree to indemnify, defend, and hold Gaggle.go LLC (dba Boppn) and the Released Parties harmless from and against any and all claims, losses, liabilities damages, costs and expenses arising from or in any way incurred or connected with any injury and/or property damage that I may cause or sustain while participating in the Boppn Scavenger Hunt Event. I understand and agree that neither Gaggle.Go LLC (dba Boppn) nor the Released Parties are responsible for any harm in any way whatsoever that I may incur as a result of my participation in the Boppn Scavenger Hunt Event. I attest that any consumption of alcohol is my own choice, and neither Gaggle.Go LLC (dba Boppn) nor the Released Parties are responsible for any harm in any way whatsoever that I may incur as a result of consuming alcohol throughout the Boppn Scavenger Hunt Event. I am aware all activities that are a part of the Boppn Scavenger Hunt Event are completely voluntary and there is no specific task or challenge that must be completed in order to win. In the event I am unable to participate, I understand there are no refunds for my Venmo payment. MEDIA RELEASE / CONSENT In consideration of my being allowed to participate in the Boppn Scavenger Hunt Event (and for other valuable consideration which I acknowledge receiving), I grant permission to Gaggle.Go LLC (dba Boppn) to take audio, video, photographic and other recordings of me and to use such recordings and my name, likeness and voice/sound in perpetuity anywhere in the world and in all types of media (including any new forms developed after the date hereof). I understand that I will not receive any compensation for the use of the audio, video, photographic and other recordings of me from Gaggle.Go (dba Boppn). I release Gaggle.Go LLC (dba Boppn) and the Released Parties from all claims by me or any person acting on my behalf in connection with this consent. I agree that the consent and release given by me in this form cannot subsequently be revoked by me. I am over 18 years old (the legal age of consent) and in my right mind and not under the influence of any substance or force. By my approval below, I agree that I have read this document carefully before approving, that I fully understand the meaning and impact of this document (including, without limitation, the assumption of risks, waiver, release, indemnity and consents) and that I agree to all of the terms and provisions set forth.uu
